Before a patient is released, the experts in the Care Navigator program talk to doctors and discuss potential needs. Then, after a personalized plan is made for each patient, the patient may return home with Care Navigator assistance. Care Navigator agents check your home for fall risks as well as monitoring diseases, ensuring you take your medicine and keeping track of doctor appointments.
